Steps To Membership:

1. APPLICATION FORM: You may get an application by calling the FDA at (800) 877-9922 or by downloading and printing a PDF application form. Once you have returned the application to the FDA, the membership process will begin.
2. DUES STATEMENT: When the FDA receives and verifies your application, you will receive a dues statement. The dues payment you make as a member is a small sum for the services that the ADA, FDA and FDA component societies provide, and for the unity of purpose you gain in organized dentistry.
3. APPLICANT STATUS: Upon receiving your dues payment, you are given applicant status in the FDA, and your application is sent to the FDA office in your district for processing. Applicant status, which entitles you to all the benefits of membership except voting, is maintained for one year. During that time, you will be acquiring membership in the component association.
4. APPROVED MEMBERSHIP: Once notified of your applicant status, the component association in your area will place your name on the agenda for the next meeting of the electing body. If elected into membership, you may be required to attend an indoctrination meeting at the component level to acquaint you with the organization and its members. (Note: Acceptance into your component means membership in the Florida Dental Association.)
